Community Transition Program (CTP)
Community Transition Program
The Community Transition Program is a re-entry program for offenders from the Indiana Department of Corrections. This program places offenders on Work Release or Home Detention, then works with them to prepare them for their release back into the community .
Services provided as part of this program include:
- An assessment of the risks and needs of the individual
- Development of a Case Plan that is unique to the individual
- Case Managing the plan.
